The Jewish Home specializes in providing both long-term and short-term skilled care and services for seniors who primarily reside in Northern California, and who are 65 years of age or older. The Jewish Home welcomes and admission is open to elders of all faiths, ethnicities, and racial backgrounds.

Nursing, medical and clinical care, rehabilitation services, social services, spiritual, and recreational programs are designed to care for those in need of physical and/or cognitive support.

The Home’s intake social workers welcome inquiries and gladly take referrals, give tours, and provide information about the Jewish Home or alternative community resources. Long-Term Care

The Jewish Home has more than 420 long-term care residents. There are ten distinct living areas, including a secured environment for residents with Alzheimer’s disease and related illnesses.

Persons interested in long-term care must complete an Application for Admission. This, along with current medical records and interviews with the intake social worker and licensed nurse, provide the psychosocial, medical, financial, and legal information that comprise the admission’s assessment.

Long-term care is paid for either privately or under Medi-Cal’s long-term care benefit. Medicare may also offer limited coverage. The director of Admissions approves applicants for admission. Once an application is approved, there is a waiting period for admission, ranging from three to eighteen months.

Before admission:

  1. Complete the Application for Admission and related documents, and submit them to the intake social worker
  2. Make an appointment with the intake social worker for a psychosocial and nursing assessment
  3. Upon approval of the application, expect a three- to eighteen-month wait for an available vacancy
  4. Maintain regular contact with your intake social worker and provide information about any changes in condition
  5. Upon notice of an availability, inspect the room and schedule an admission date within three days

On admission day:

  1. New resident will be welcomed and introduced to the Jewish Home community
  2. Admission documents will be signed
  3. Clothing and furnishings will be inventoried and labeled
  4. Resident's medications will be retained by nursing staff
  5. Admission assessments will be performed by nursing and medical staff

Short-Term Rehabilitation Care

The Jewish Home offers a number of rooms for those seeking rehabilitation care of two months or less. An accelerated assessment process in these cases may secure immediate placement. Generally, payment for this type of care is covered under Medicare, Part A.

Acute Psychiatric Care

For those in need of voluntary, acute hospitalization for psychiatric treatment, the Jewish Home offers excellent care, for the general community as well as for its residents, in an intimate setting. As a Medicare, Part A and Kaiser benefit, the Home’s program offers immediate availability to those qualified for this intensive service.
